# Growth Chart

The Growth Chart feature in Unite EMR enables healthcare facilities to track and evaluate patient growth over time, using WHO standard metrics. This feature provides a visual tool to monitor critical growth parameters, such as height, weight, BMI, and head circumference. By comparing these metrics against WHO growth standards, healthcare professionals can assess a child’s growth pattern and quickly identify any potential health concerns.

### Accessing the Growth Chart

* Open the **Medical Records** screen.
* Select either the [**Diagnosis**](/uniteemr/application-modules/medical-records/diagnosis.md), [**Casesheet**](/uniteemr/application-modules/medical-records/casesheet.md), or [**Medication**](/uniteemr/application-modules/medical-records/medication.md) tab.
* In the **More Actions** menu, select **Growth Chart** to open the patient’s Growth Chart data.

* The Growth Chart pop-up is divided into two panels:
  * **Left Panel**: Displays the growth chart.
  * **Right Panel**: Contains fields such as patient gender, chart type, **Select Growth Chart** dropdown, **Edit Chart Data** section for data entry, and Entry History.&#x20;

### Types of Growth Charts

The Growth Chart feature in Unite EMR supports different growth classifications based on the patient’s age and gender, adhering to WHO standard metrics. Growth charts display pink for girls and blue for boys for quick visual distinction.

* Head Circumference-for-Age and Weight-for-Length percentiles.
* Length-for-Age and Weight-for-Age percentiles.

2. [**2 to 20 Years - Girls and Boys**](#id-2-to-20-years-girls-and-boys)

* BMI-for-age percentiles.
* Stature-for-age and Weight-for-age percentiles

#### **Entering Growth Chart Data**

To enter and track a patient’s growth data, select the respective growth chart from the **Select Growth Chart** dropdown menu. Based on the selection, the relevant chart displays and allows entry of new data.

#### 0 to 24 Months: Girls and Boys

**Head Circumference-for-Age and Weight-for-Length Percentiles**

* In the **New Entry** section, enter/select the **Entry Date** using the date picker. The **Age** field will auto-calculate based on the Date of Birth (DOB) provided in the Patient Register.<br>

* **Head Circumference**: Defaults to centimeters; you may change to inches if needed. Enter the head circumference in the provided field.
* **Length**: Defaults to centimeters; you may change to inches if needed. Enter the length, or if vitals are captured, the height will automatically populate.
* **Weight**: Defaults to kilograms; you may change to pounds if needed. Enter the weight, or if vitals are captured, the weight will automatically populate.
* **Remarks** (Optional): Enter additional notes if needed.
* Click **SAVE** to finalize and store the entry. A blue plot on the chart indicates an entry in progress for girls, and a red plot indicates an entry in progress for boys. After saving, the plot changes to black and appears in the data table below.\

#### **Length-for-Age and Weight-for-Age Percentiles**

* When selecting this chart, the **Head Circumference** field will be disabled.
* Enter/Select the **Entry Date** using the date picker. The **Age** field will auto-calculate based on the Date of Birth (DOB) provided in the Patient Register.<br>

#### **Viewing, Editing, or Deleting Entries**:

* Saved entries are listed in the table below, with details such as **Entry Date**, **Age**, **Weight**, **Length**, **Head Circumference**, and **Actions**.
* Hover over each entry to reveal options for **Edit** or **Delete**.
